- Assessment Visit – Once a referral is received, we schedule a suitable date to visit the prospective resident for an assessment. This allows us to better understand their needs, preferences, and overall wellbeing.
- Initial Care & Support Planning – If the placement is deemed suitable, an initial care and support plan is developed. This plan is tailored to the individual’s needs and helps determine the appropriate funding arrangements.
